You do not have to complete your health questionnaire prior to taking your test, however we recommend completing the health questionnaire before your GP results appointment.
The more information we have on your health history, the more personalised your consultation will be. Some questions which are essential for us to be able to make the service tailored to your needs but other questions about your past or personal history can be skipped and reviewed at a later date if you wish to do so.
